L.A. County-based public health information exchange enables sharing of patient records between safety net providers for care coordination
LOS ANGELES, Calif. – June 20, 2017 – Glendale Adventist Medical Center and White Memorial Medical Center are partnering with Los Angeles Network for Enhanced Services (LANES) to deliver better, more efficient health care to the communities they serve. Through the collaboration, the hospitals executed a contract with LANES to support secure information exchange between hospitals, community-based clinics, and health plans in Los Angeles to support better care coordination – with special emphasis on underserved populations.
